Hospital or Medical Facility

We understand that these are difficult times on the families as well as the patients and we strive to make this process as stress free as we can. In order to ensure we make this a seamless transaction there are a few requirements to follow.

• Signer must be alert and oriented 
• Medication that may have negative effects on cognitive function should be held for 1 hour prior to signing. 
• If signs of coercion are present we may request non-essential medical professionals clear the area. 
• We ask that you remain mindful of your loved ones caregivers and schedule appointments during a time that is best for your patients care. 

• ID verification of the signer must be valid 
  • Passport, Driver's License, Government issued identification card.
  • Contains the signature and photo of the singer. 

Please ensure you have all necessary documents and valid proof of identification ready prior to our arrival.
